In my 48-year software career (which is still in progress) I've been a programmer, designer, architect, VP, CEO, and founder. In my opinion, most companies fail to manage software development effectively. Recently that has been made worse by the growing adoption of pseudo-Agile methodologies, like Scrum, that turn the process into a hamster wheel for developers - when the real need is for competent communication and leadership.
